For this tutorial you will need:
A Tube, colours and font of your choice.
I have included the materials I used in the zip file

Open a New Transparent Image 650 x 450
Flood Raster 1 with #a5b5a5
Copy and Paste mc920 face tube as a New Layer (minimize, we need this again)
Effects - Image Effects - Seamless Tiling
Corner checked, Bidirectional checked, Horizontal and Vertical Offsets = 0, Transition = 100, check Linear corner style
Change the Layer Opacity to 60
Same Layer
Adjust - Blur - Radial Blur, Check Spin, Blur Strength = 50, Elliptical checked, Center - all three values = 0
Merge All Layers visible
Duplicate - Image - Resize
Change to Pixels - Uncheck Lock aspect ratio - Uncheck All Layers. Width = 425, Height = 400
Change Layer Opacity to 20 and Layer Mode to Difference
Choose Warp Brush from the Tools menu
Mode = Noise, Size = 30, Hardness = 50, Strength = 100, Step = 5, Noise = 5, Edge Mode Fixed
Hold down Shift - Click in top left corner - click in top right corner - release Shift and apply
Hold down Shift - Click in top right corner - click in bottom right corner - release Shift and apply
Hold down Shift - click in bottom right corner - click in bottom left corner - release Shift and apply
Hold down Shift - click in bottom left corner - click in top left corner - release Shift and apply
The edges will look something like the screenshot below
Click on Lock Transparency
Effects - 3D Effects - Cut Out - Vertical and Horizontal = 5, Opacity = 100, Blur = 3, Shadow colour #5eb9bc
Repeat Cut Out with Vertical and Horizontal at Minus 5
Click Lock Transparency again
Using the lock allows the effect to be applied on the same layer without losing the image
Duplicate this layer twice for a total of three
Highlight Copy of Merged - Resize this layer only 85%
Remember to change from pixels to percentage
Image - Free Rotate - 15 Right
Highlight Copy (2) of Merged
Resize this layer only 85%
Image - Free Rotate - 15 Left
Effects - Texture Effects - Weave
Gap Size = 1, Width = 5, Opacity = 10. Weave and Gap colour White. Fill Gaps checked
Apply to Copy of Merged Layer, Copy (2) of Merged Layer and Copy (3) of Merged Layer
(Edit - Repeat weave effect is a quick way of applying to all three layers - or use the repeat tool from the tool bar)
Copy and Paste mc920 face tube as a New Layer
Use the Mover tool to place against the left edge
Change the Layer Opacity to 30 - Layer Mode to Burn
Duplicate and Mirror
Merge All Layers Visible
Add Noise 8%, Gaussian and Monochrome checked
Copy and Paste mc920 as a New Layer
For the tube supplied …..
Effects - Image Effects - Offset. Horizontal offset Minus 15, Vertical Offset = 0, Transparent checked
Selection Tool - Custom Selection (for the supplied tube)
Top = 250, Left = 295, Right = 395, Bottom = 300
Selections - Promote Selection to Layer
Selections None
Move the lips to top left corner - See tag
Selections - Select All - Selections Float - Selections Modify - Select Selection Borders
Outside checked, Border Width = 1, Antialis checked
Flood the border with gold gradient or gold bead pattern
Selections None
Duplicate the lips layer
Effects - Image Effects - Offset
Horizontal = 0, Vertical Minus 100 - Transparent checked
Duplicate - Repeat Offset
Duplicate - Repeat Offset. (4 lip layers)
Hide the Merged layer and Raster 2
Merge the four lip layers visible
Unhide all layers
Change the Layer Mode on the lips to Luminance (L)
Apply Drop Shadow to lips
Vertical and Horizontal = 8, Opacity = 35, Blur = 15, Colour Black
Repeat with Vertical and Horizontal at Minus 2, Opacity = 35, Blur = 15
New Layer
Selections - Custom Selection
Top = 275, Left = 550, Right = 610, Bottom = 375
Flood the selection with gold beads or gold gradient
Selections Modify - Contract by 2 - Delete
Selections Modify - Contract by 2 - Flood with gold beads or gradient
Selections Modify - Contract by 2 - Delete
Selections None
Duplicate
Effects - Image Effects - Offset - Horizontal = 20, Vertical = 35, Transparent checked
Merge Down
Apply Drop Shadow
Vertical and Horizontal = 1, Opacity = 30, Blur = 2, Colour Black
Add text of choice in top right corner
Merge All Layers Visible
Image - Resize 85% - All Layers checked - Lock Aspect Ratio checked - Copy
Add Borders - 15 Symmetrical - Colour White
Magic Wand - Select the border and Paste into Selection. Leave those ants marching
Adjust - Blur - Gaussian Blur - Radius = 25. Leave selection active
Apply Blinds. Width = 2, Opacity = 20, Colour Black - nothing checked
Selections Invert
Drop Shadow
Vertical and Horizontal = 2, Opacity = 50, Blur = 10, Colour Black
Repeat with Vertical and Horizontal on Minus 2
Selections None
Image Add Borders - Symmetrical = 1, Colour Black
Add watermark and any necessary Copyright information
Resize using Bicubic Resampling
